Our Mission

Since 2007, ADVault’s mission is to make sure everyone can create, store and share a quality advance care plan that is accessible when and where needed.

Sounds simple, right?

  • Who speaks for you if you can’t?
  • What are your goals, preferences and priorities for care?

Everyone deserves to live with confidence they can have a voice in their own care.

That’s our definition of a real person-centered healthcare system!

After years of research, beta testing and support from some of the healthcare industry’s most innovative leaders, MyDirectives went live in 2012. Now with consumer users in over 50 countries, MyDirectives and MyDirectives MOBILE is the world’s leading all-digital advance care planning platform.

MyDirectives lets people create, store, update and share the free MyDirectives digital advance care plan or upload any third-party digital or paper-based advance directive or advance care plan such as documents from Caring Conversations®, Five Wishes®, the VA form 10-0137 (U.S. Department of Veterans Affairs), or portable medical order (POLST) document. The ADVault Exchange integrates with electronic health records, health information exchanges, payers and providers to allow access 24/7, anywhere in the world.
